Each 4-H'er who was a member of the club last year should receive a re-enrollment email from firstname.lastname@example.org. Parent volunteers should receive one as well in order to log on and re-enroll.
To complete the enrollment process, please complete both a Health History (HH) and a Code of Conduct (COC) and either mail it (address provided in an email to club members) or turn it in at the next club meeting or activity your child attends. Please ensure the forms are completely filled out (including the Media Release section) and signed by both a parent/guardian and the 4-H member. Links to the forms are below.
Krazy for K9s has one final requirement for enrollment: we must have on file documentation to verify that your dog's rabies vaccination is current. Please check with the club registrar if you have any questions.
Code of Conduct Form
If you had also "enrolled" as a parent (and we ask that at least one parent enroll for each 4-Her), you should have received an email with an OpenID for you too. Please re-enroll as directed on the website.
Please let club leader Sheila Baugh know if you did not receive the email from "email@example.com" with your 4-Her's (and your) OpenID or if you have any questions regarding enrollment.

If you are new to 4-H, go to the bottom section "join 4-H" and begin by selecting VA . On the next page, you will select "Fairfax" as the county. The enrollment form should open next. Complete the form. PLEASE include phone numbers so that we know how to reach you in case of emergencies. When you come to Club Memberships, start typing "KR" in the space and select Krazy for K9s. Then right below that is Projects. If you type "dog" in the box, a list appears. Select all except "national". Unfortunately, you have to select them one at a time and then enter "KR" in the box below that for "club" and select Krazy for K9s each time -- after each one, click on "add project" just beneath the club field to get a new set of boxes for the next "dog" project selection.
Please fill out the enrollment as best you can. There will be questions but there will be a chance to edit the enrollment later to make any corrections. You do not need to print out the form, but you can if you want a hard copy.
Once the 4-Her's form is completed, please go back to the original enrollment link and enroll yourself as we do ask that at least one parent per child enroll too. This time after you select VA, then go to the next page, select Fairfax, then please click the "volunteer" box. On the enrollment page, complete the form and please include phone numbers and select Krazy for K9s under club. No need to do the projects section.
When you are finished, please send leader Sheila Baugh an email and let her know your child's name and your name.

Please submit the following forms to the Krazy for K9s Club Adult Leader Sheila Baugh if you wish to join the club. If you wish to enroll electronically please follow the directions in the boxes below for new or returning members respectively, which replaces the 4-H Youth Member Enrollment Form. We invite you to visit our meetings and training sessions for a couple weeks before deciding to enroll. Each member must complete these forms annually:
